Physician Information

Spectrum pain management physicians see patients at either of the following locations:

Please complete the patient referral form for the location the patient will be seen at, below, for each new referral. The referral form should be faxed back to us as soon as possible so that we may schedule an appointment for your patient.

Referring physicians may speak with a Spectrum physician by calling either telephone number listed above.

Patients with chronic headaches may be referred to us if there is a diagnosis of whiplash or spinal pathology; all other patients are best treated by a neurologist. Please note that we are not licensed to provide opiate detoxification.

Patient Information

Your physician must make a referral to the pain clinic. Once he or she has completed and faxed your referral information sheets, a Spectrum pain management physician will review the information and determine if an assessment should be scheduled for you at the clinic. Our staff will contact you to arrange an appointment once all of these steps have been completed. At that time, you should complete both the Authorization to Disclose Information Form and the patient referral form, below, for the location that we will be seeing you at. You should bring these forms with you to your appointment.
