Bivalirudin (AngioMax) 

-for anticoagulation in the presence of HIT (Heparin-Induced Thrombocytopenia)

Use on CardioPulmonary Bypass, CPB (from Univ. of Michigan)

For Non-Cardiac Surgery

  • Initial Bolus syringe 0.5 mg/kg
  • to maintain desired ACT (usually 1.5-3x baseline)
  • Infusion: 1 mg/kg/hr (titrated to ACT)
  • If on HemoDialysis: 0.25 mg/kg/hr (titrated to ACT)
  • Continue infusion until 15 min. prior to anticipated end
  • Infusion may need to be decrease in renal insufficiency or hypothermia, monitor ACT closely
