Spectrum Medical Group office hours are 8:00am to 5:00pm, Monday through Friday. A consultation or follow-up visit may be booked by calling 207-347-2660. If you ever feel you are experiencing a medical emergency, do not hesitate to call, regardless of day or hour.

When you call for an appointment, the scheduler will ask for the following information:

• Patient Name
• Date of Birth
• Address
• Home Telephone
• Work Telephone
• Brief description of the reason for the appointment
• Primary Care Physician
• Insurance or HMO information

Spectrum will send a confirmation letter with the date, time, location, instructions and forms for your appointment. Spectrum's scheduler will call you 1-3 days in advance of your appointment. Please read and complete the following forms prior to your appointment:

• Spectrum Medical Group Notice of Privacy Practices

Preparing for your office visit

Please assist us by bringing the following items to your appointment:

• Patient Medical History Questionnaire
• Patient Information Form
• Referral Form (for HMO Insurance if required)
• Insurance Cards
• Relevant information about your medical and surgical history (if not forwarded by your doctor)
• Most recent imaging films and reports (x-rays, CT, MRI, US)
• Most recent laboratory study

Cancellation or Rescheduling Appointment

Please call us as soon as you realize you must reschedule or cancel. To allow other patients the opportunity to take your appointment, notify us within 48 hours of your appointment. If you are running late for your appointment, please call the site where you are schedule to provide your estimated time of arrival.
